What is the POC policy regarding insurance?

We are contracted providers for the following insurance carriers as of 2008:

Blue Cross, Providence Health plans, Cigna, ODS, MHN, Pacificsource, Medicare, Great West HMO, Aetna (this may change for 2009). 

Note:  We do not accept any Oregon Health Plans.

Deposits: We do require a $300.00 deposit at your initial visit. We may require more if your responsibility will be greater than that, based on your insurance benefits.

For persons that are not covered by these insurance carriers, we are on a fee-for-service basis, with payment in advance. We take VISA and Mastercard.  Be sure to call our office at (503) 233-5925 if you are unsure about whether your insurance is applicable.  

If I am not covered by one of the above insurance carriers, what is the cost of the Comprehensive Vertigo Consultation, including the tests?

The average cost is approximately $2100.

If my condition proves to be treatable, what is the cost of treatment?

Cost of treatment after the consultation will range generally between $2000 - $6000.
